The short version. This site collects your answers and email to notify you when our doctor-supervised weight-loss program opens. Nothing on this site is medical advice, no medication is offered or sold here, and any future prescription decision belongs entirely to a licensed healthcare provider. You must be 18+.
1. What this site is
WellPath is a pre-launch informational site. The eligibility quiz is a screening questionnaire, not a diagnosis, consultation, or guarantee of treatment. Completing it creates no patient-provider relationship.
2. Not medical advice
Nothing here diagnoses, treats, or prevents any condition. Prescription products, if offered at launch, will require evaluation by an independent licensed healthcare provider who alone decides whether a prescription is appropriate. Many applicants will not qualify. Talk to your doctor before starting any weight-loss program, and do not use this site in a medical emergency.
3. No guarantees
We make no promises about eligibility, availability, pricing, or results. Weight-loss outcomes vary substantially between individuals. Any figures shown are illustrative program information, not predictions.
4. Eligibility
You must be at least 18 and located in the United States. This program is not for use in pregnancy or while nursing.
5. Privacy — what we collect and do
- We collect: your quiz answers (goal, height, weight, age, conditions, state) and your email address.
- We use them to assess interest, notify you at launch, and improve the site.
- We do not sell your personal information.
- Hosting and form processing are provided by Netlify; analytics/advertising pixels (e.g., Meta) may record site interactions as described in their policies.
- Email privacy@wellpath.health to access or delete your data.
6. Communications
By submitting your email you agree to receive launch-related emails from us. Every email includes an unsubscribe link; unsubscribing is immediate.
7. Acceptable use & IP
Don't scrape, disrupt, or misuse the site. Site content is ours; you may not reuse it commercially.
8. Disclaimer & limitation of liability
The site is provided "as is," without warranties of any kind. To the maximum extent permitted by law, our total liability arising from your use of this site is limited to $100. We are not liable for indirect, incidental, or consequential damages.
9. Changes; governing law
We may update these terms; the date above will change. Continued use is acceptance. Governed by the laws of the State of Delaware.
